I've commented on your form in Google docs. I said: This is basically fine, but strictly speaking you should provide a brief information sheet about the project, with details such as how much you want to record, what will happen to the recording, who will hear it, assurances about where it will end up (usually a guarantee to destroy it after a certain period of time) etc. There is a lot of detail about all this on the RSEL wiki, under Milestone 2 / data collection.
